This notice explains what information Glivara Clinical collects through glivaraclinical.com, why, and the choices you have. Glivara Clinical, LLC ("Glivara Clinical") is a pre-launch clinical consulting company serving obesity-medicine practices.
This site is not for patient information
Our forms ask for practice-level input only. Please do not submit patient names, health details, or any other protected health information. If you include it by mistake, email us and we will delete it.
What we collect
When you use a form on this site, we collect what you enter:
- your name, professional role or credentials, professional email, and (optionally) your practice or organization;
- your answers to our practice-friction question, including any short write-in answer;
- your choices about our newsletter and about a founding-clinic conversation, and the date and time of those choices.
Like most websites, our hosting provider also processes basic technical information, such as your IP address and browser type, to deliver and protect the site. We use Cloudflare Web Analytics, which counts visits without cookies and without tracking you across other websites. We do not use advertising cookies or sell your information.
How we use it
- Shaping the newsletter. Responses help us choose topics for The AOM Practice Brief. We may describe themes in aggregate, but we will not publicly identify you or your practice.
- The newsletter. We email The AOM Practice Brief only if you check the box asking for it and confirm your subscription. Every issue includes an unsubscribe link.
- Founding-clinic conversations. If you check that box, we may email you once to ask whether you would like to talk. Checking it is not a commitment of any kind.
- Replying to you when you contact us.
Who processes it
We use ActiveCampaign to collect form submissions, manage newsletter subscriptions, and send email, and Cloudflare to host this website. These providers process information on our behalf under their own security and privacy terms. We do not share your information with anyone else except where required by law.
Your choices
- Unsubscribe from the newsletter at any time using the link in any issue.
- Ask us to access, correct, or delete your information by emailing hello@glivaraclinical.com.
How long we keep it
We keep your information while you remain subscribed or while it is useful for the purposes above, and delete it on request.
